Job summary

We’re looking for a Band 6 Mental Health Practitioner to join our Community Mental Health Service in Somerset. This is an exciting opportunity to work within a progressive, recovery-focused team that puts people at the centre of everything it does.
In this role, you’ll support adults experiencing severe and enduring mental health difficulties, providing high-quality assessments, therapeutic interventions, and personalised recovery planning. You’ll work alongside a wide network of health, social care, voluntary sector and community partners, helping individuals access the right support at the right time.
This is a fantastic opportunity to further develop your clinical expertise, contribute to service improvement, and play an influential role within a supportive multidisciplinary team.
You must be a car driver with access to transport for work purposes. Community mental health experience would be advantageous.
Main duties, tasks & skills required
As a Mental Health Practitioner, you will:
- Manage a varied caseload of adults with complex mental health needs within the community.
- Complete comprehensive mental health and risk assessments using a strengths-based, recovery-focused approach.
- Develop, implement and review personalised recovery and risk management plans.
- Deliver evidence-based psychosocial interventions that promote independence, wellbeing and resilience.
- Build effective therapeutic relationships with service users, families and carers.
- Work collaboratively with colleagues across health, social care, primary care and voluntary sector organisations.
- Support individuals through periods of crisis, relapse and recovery while promoting positive outcomes.
- Act as an advocate for service users, ensuring their voice remains central to care planning and decision-making.
- Contribute to clinical leadership within the team through supervision, mentoring and supporting less experienced colleagues where appropriate.
- Participate in service development, quality improvement and clinical governance initiatives.

Qualifications
Essential
- RMN, Social Work or Occupational Therapy Qualification
- Current registration with relevant professional body
- Evidence of post qualifying professional development in an acute setting.
Experience
Essential
- Significant post registration experience in multi-disciplinary community mental health services.
- Experience of managing risk of clients in crisis.
- Experience of assessment, care-plan, development and evaluation in conjunction with clients and carers.
- Experience and knowledge of working with people with mental health problems.
Desirable
- Experience of mental health difficulties.
