Summary:
The Clinical Records Reviewer is responsible for conducting detailed reviews of medical records to evaluate appropriateness and quality of care, identify adverse events, hospital-acquired infections, and potential surgical misadventures. This role involves reviewing claims from a wide variety of admissions, including NICU, cardiac, trauma, oncology, burns, and general surgery. Each case requires comprehensive review of all clinical documentation for the admission, with key findings accurately captured in a spreadsheet.
Duties and Responsibilities:
· Review complete clinical records for various admission types to assess quality of care and identify adverse events or discrepancies.
· Evaluate documentation related to NICU, cardiac care, trauma, oncology, burns, and general surgery admissions.
· Detect hospital-acquired conditions, surgical misadventures, medication-related issues, and gaps in care.
· Summarize clinical findings and enter clinical findings into a spreadsheet with accuracy and consistency.
· Assess the appropriateness, completeness, and clarity of clinical documentation.
· Identify patterns, issues, or inconsistencies that may impact quality of care or claims outcomes.
· Communicate observations and findings effectively with internal team members or other stakeholders as needed.
· Maintain strict confidentiality and adhere to all privacy and compliance regulations.
Required Skills and Abilities:
· Healthcare documentation specialist experience (transcription/scribe, auditing, or correction of dictated electronic health records) with a minimum of 6 years of multi-specialty acute care experience (QA experience a plus), OR healthcare data abstractor experience in an acute care setting.
· Extensive knowledge of medical terminology, anatomy, physiology, disease processes, pharmacology, imaging, and laboratory studies.
· Strong understanding of clinical record structure and the ability to interpret and summarize complex medical information.
· Exceptional attention to detail with strong ability to identify problems and employ problem-solving skills.
· Ability to identify discrepancies, inconsistencies, or documentation gaps effectively.
· Strong written and verbal communication skills.
· Basic working knowledge of Microsoft Excel (data entry, sorting, filtering, basic functions).
Education and Experience:
· Multispecialty acute care transcription background mandatory (minimum 6 years).
· Experience with clinical quality review and medical claims evaluation.
· Familiarity with hospital workflows, standards of care, and documentation practices.
Physical Requirements:
· Ability to work independently while managing multiple cases with accuracy and efficiency
· Individuals in this position must be able to work in a standard office environment which requires sitting and viewing monitor(s) for extended periods of time, operating standard office equipment such as, but not limited to, a keyboard and telephone.
