The role:

Townsend Intermediate Care Hub is a CQC registered 35-bed reablement unit based in the North-central area of Liverpool. It aims to assist individuals in their transition from hospital to home by helping them improve and retain their independence through therapy, social interaction, and reablement.

The role of a reablement assistant involves working with service users within a reablement environment to provide support with their care needs, and to help them attain an agreed level of independence

Working with social workers, occupational therapist, and physiotherapist, you will be required to assist and empower service users with their domestic and personal activities of daily living

The role of a reablement assistant requires you to be able to record with care those activities and actions undertaken with the service users, so that we can clearly demonstrate our commitment to them and their care

The role can be physically and mentally demanding depending on the needs of the individuals within our care
